Why follow-ups fail in most hospitals

A follow-up is advised in the consultation room and then handed to nobody. It is written on a prescription, maybe in a register, and the patient is expected to remember. When the front desk is busy, a staff member is on leave or the patient simply forgets, the review never happens. Nothing in a typical HIMS flags it, because the HIMS records visits that happened, not visits that should have.
